Abstract
A 21- year-old girl with a normal body mass index presented with acute onset pain abdomen, vomiting and loose stools since one day with no significant past history. She was pale however her vitals were stable and systemic examination was normal. She was treated symptomatically however keeping a high index of suspicion for acute surgical abdomen radiological imaging was done which showed a large right hemorrhagic paraovarian cyst for which she was referred for laparoscopic surgery. The final diagnosis after surgery was paratubal cyst and acute appendicitis which was removed and the girl recovered completely.Paratubal cyst is in itself a rare reportable entitity in a young unmarried girl and being seen together with acute appendicitis makes it a remote case.We concluded that multiple surgical pathologies may coexist and should be kept in mind while examining young adults with acute history of pain abdomen
